Parents,
We made a mistake with our medical forms and some of them were not signed correctly. To correct this situation, we are sending out forms with students today. Not all students will receive an envelope, only the ones that were not correct.
Please check with your child to see if they received a letter and return these as soon as possible. We apologize for the mistake or any confusion that this may cause.
